2018 Subcontractor Standard Payment Process
W-9 Form
Current Certificate of Insurance, including the following:
- Worker’s Compensation / Employer’s Liability
  1. Statutory Limits: $1,000,000 Bodily Injury Each Accident
  2. $1,000,000 Bodily Injury by Disease Each Employee
  3. $1,000,000 Bodily Injury by Disease Policy Limits
- Commercial General Liability (typically issued on an occurrence basis):
  1. Bodily Injury / Property Damage, $1,000,000 each occurrence
  2. General Aggregate, $2,000,000
  3. Contractual Liability, $2,000,000 each occurrence
  4. Personal & Advertising Injury, $1,000,000 for each occurrence
  5. Products / Completed Operations, $2,000,000 per project aggregate
  6. Premises Damage Legal Liability
  7. $1,000,000 each occurrence
  8. Medical payment to any one person $1,000,000
- Commercial Automobile Liability (if required by Skyler):
  1. Combined Single Limit, $1,000,000 for each accident
- Additional Requirements for all MEP subcontractors and any subcontractor with aggregate payments exceeding $1,000,000:
  1. $3,000,000 each occurrence
  2. $5,000,000 general aggregate
- Umbrella or Excess Liability Insurance (occurrence basis, if required by Skyler):
  1. Bodily Injury / Property Damage, Combined Single Limit, $2,000,000 each occurrence and $2,000,000 aggregate
- Contractor’s Professional Liability (if required by Skyler):
  1. $1,000,000 plus $1,000,000 Excess – Project Specific
- Architects / Engineers Professional Liability (if required by Skyler):
  1. $2,000,000 plus $2,000,000 Excess – Project Specific
  2. Coverage maintained for five (5) years following Substantial Completion
